Community services for older people
Sage provide support and advocacy for older people who live in their own homes. Regularly older people need support to remain in or return to their home. This is especially true after a period spent in hospital or in a long-stay residential facility.
Sage also supports groups of older people who want to improve the ways in which their services and entitlements are provided in their locality.
Sage builds Circles of Support & Advocacy to maintain older people in their homes. A Circle of Support promotes effective linkages with and between primary, acute and residential care providers. Transitions into other care settings are eased by strengthening the natural supports of family and community.
