AgeWell Companion

How can I become an AgeWell Companion?

AgeWell is a Peer to Peer Care Model which engages able adults 50+ to visit less able older people in their homes to:

  • Reduce isolation

  • Identify evolving health problems

  • Link older people to appropriate Primary Care Providers and Social and Community Services

The Care Model strives to safely and conveniently connect dedicated companions (AgeWells) with older people in their local communities. As a companion, you are trained to collect critical information on clinical, social and environmental issues during your regular visits which in turn trigger responses into action items that promote good health and trigger referrals to clinical, social and community services.

Training

AgeWell Companions receive training to become part of a team of companions offering one- to-one companionship and community support to older people in their own homes. These older people may be people living with an illness, socially isolated or living alone or simply in need of a chat each week.

Learn how to forge healing relationships through conversation and engaged listening:

  • As a companion, you will meet fellow companions weekly to share experiences on ways to encourage Programme Participants, by encouraging them to reconnect to their community.

  • Learn about the simple power of conversation and human connection in promoting health and well-being for our Older People.

  • As a companion, you will also support Program Participants as they re-establish ties to their community; This is important because connecting to the community plays a key role in promoting mental health and combating loneliness.

  • This is accomplished through weekly visits and also by encouraging participants to utilise and seek out community resources, and by encouraging them to connect with other people.

  • Provide companionship; ensure that the dignity, privacy and independence of the Older Person is respected at all times. Be mindful of the ethics and culture of the organisation and the role played supporting our older people.
